چکیده

Calcific uremic arteriolopathy (CUA) [1] refers to calcification of the media of small terminal arteries and arterioles and associated fibrotic intimal thickening and lumen narrowing, thereby increasing the risk of ischaemic necrosis. As the term suggests, CUA is reported in patients with chronic kidney disease (CKD) ‘nearly exclusively’ [2]. Identical clinical and pathologic features have developed with hyperphosphataemia from other causes [3–7]. CUA develops silently [1,5,8], sometime presenting only with subcutaneous plaques and/or nodules. But with ischaemic complications it presents acutely with foci with discoloured skin progressing to necrosis and deep ulcers; hereafter designated complicated CUA. Once mistakenly likened to an allergic reaction by Selye (and therefore inappropriately termed ‘calciphylaxis’ [9]), complicated CUA is uncommon but is recognized increasingly in dialysis patients and renal transplant patients [3,8]. The lesions have appeared in ‘proximal’ body areas – abdominal wall, thighs, flanks, buttocks and breasts – and are generally more devastating than ‘distal’ ones localizing mainly in the lower legs [1,5,9–12]. Both may coexist [5].

Breast pain in a patient on dialysis: a rare manifestation of calcific uraemic arteriolopathy.

A 63-year-old woman presented with progredient bilateral breast pain. Her medical history included rheumatoid arthritis, AA amyloidosis and end-stage renal disease treated by peritoneal dialysis. Inflamed skin alterations of the breast and laboratory values suggested mastitis non-puerpuralis but antibiotics did not resolve the symptoms. Sonography and mammography showed severe vessel calcificat...

Calcific uraemic arteriolopathy: local treatment and hyperbaric oxygen therapy.

Sir, Calcific uraemic arteriolopathy (CUA) (‘calciphylaxis’) is a rare necrotizing cutaneous abnormality which is being increasingly recognized in the dialysis population w1x. It is usually associated with secondary hyperparathyroidism in the presence of advanced renal failure and high calciumu phosphate product w2x. Treatment is difficult and often unsuccessful and is associated with a high mo...

Hyperbaric oxygen therapy in the treatment of skin ulcers due to calcific uraemic arteriolopathy: experience from an Australian hyperbaric unit.

INTRODUCTION Calcific uraemic arteriolopathy (CUA), also known as 'calciphylaxis', is a syndrome of ischaemic necrotic ulcers occurring in uraemic patients with end-stage renal failure. It is a debilitating condition with a high morbidity and mortality. Hyperbaric oxygen (HBO) has been used to treat such wounds for many years but evidence of its efficacy is limited. AIM We aimed to study the ...

Successful treatment of calcific uraemic arteriolopathy with bisphosphonates.

BACKGROUND AND OBJECTIVES Calcific uraemic arteriolopathy (CUA), also known as calciphylaxis, is a rare but life-threatening condition that almost exclusively affects patients with chronic kidney disease. Several therapies have been employed to treat this disease but with irregular results. We report a prospective case series of eight patients diagnosed with CUA in our unit between 2002 and 201...

Vascular calcification in chronic renal failure.

The prevalence and extent of vascular calcification (VC) increases rapidly with time on dialysis. There is increasing evidence that medial calcification of conduit arteries, without intimal disease, is associated with important abnormalities of vascular compliance and increased risk of cardiovascular death. Coronary artery calcification is also common in end-stage renal disease, but further res...
